On this page you will find all of our adventurer resources from sign up forms, copies of our policies and procedures and some information brochures from other services that we recommend for advocacy etc.

We have also provided instructions and links on how to submit some feedback or a complaint to us.

Referrals

If you are from the LAC or a Support Co-ordinator (aka “little Angels”), you can complete the sign up forms below on behalf of your participants, or complete the online referral form here. Once we have basic information we can then co-ordinate a meet and greet via you or directly with the participant / their carer, just let us know when you send the forms in.

You can also just give us a call on 0480 185 840 and have a chat to us about your participant and we can send you all the relevant forms via email and book meet and greets over the phone.

Sign Up Forms

So that we can provide the best care possible and help reach your goals, we ask you to complete our Sign Up forms. The forms are quite in depth and not all sections may be relevant to your needs so please just fill out as much as you can and if you have any queries don’t hesitate to get in touch with us.
The form and reading materials you will need to begin your adventures with us are listed below. Please take your time to read through all of the documentation so you are aware of your rights and our responsibilities before we get started.

If you have any problems accessing the files please contact us or call us on 0480 185 840 with your mailing address and we will send you out a printed pack instead.

Once we have received your completed forms we will use some of that information to create a service agreement which we will all sign and then the adventures can begin.
